Supported Living

Supported Living is a service delivered to people in their own homes within their community as oppose to at a residential setting or at home with their family. We support individuals who have their own place or that share their home with others.

Who is supported living for?
Supported living is for adults with a learning disability who would like to live independently in their own home but require support with day to day tasks.

These tasks can include support with:

  • Personal care
  • Taking medication
  • Managing finances
  • Maintaining contact with families and friends
  • Accessing the community
  • Cooking
  • Maintaining household tenancy
  • Day to day household tasks


Each supported living package is bespoke to each person to cater for their individual. The aim of the service is to allow each individual to live independently in a safe and supported environment, where all their needs are met.

Supported Living packages can be accessed through Pembrokeshire County Council and can be funded by a local authority, with private funds, with a personal budget or a direct payment.

 

Domiciliary Care

Living at home independently is an ideal for most people to do for as long as possible. In some cases, support is sometimes needed to allow people to achieve this. Domiciliary care provides that support. However the best person to identify the support that is required is you. Support will be provided by a skilled and experienced team of support workers with local knowledge of your area. Support can be provided 24/7 and you will be delivered with a person centred ethos. Support will be specific to your needs.

Domiciliary support can be provided for:

  • Everyday tasks
  • Personal care
  • Support with information
  • Advice
  • Medication management
  • Hospital to home services
  • Respite care
  • Live in care
  • Accessing employment or education
  • Companionship
  • Accessing the community
  • Accessing social and leisure activities

Domiciliary care can be provided throughout Pembrokeshire and can be accessed both privately or funded by Pembrokeshire County Council (dependant on circumstance). Each care package will be specific to the individual in order to meet their specific needs.
